The honest answer is: it does specific things well, and it does not do other things at all. Understanding the distinction is important before committing to a course of treatment.

What the CUA & AUA Say

The Canadian Urological Association guideline for Peyronie’s disease acknowledges shockwave therapy as a supported modality for pain management and as a component of multimodal treatment. The guideline does not recommend shockwave as a primary intervention for curvature correction. This is a clinically important nuance: a treatment can be evidence-supported and guideline-acknowledged for specific outcomes without being effective for all outcomes.

The AUA guideline for Peyronie’s disease reflects a similar position. Shockwave is recognised for its role in pain management and as a reasonable adjunct, while the guidelines are explicit that curvature correction in the stable phase is best addressed surgically in candidates who meet criteria.

Where Shockwave Fits in a Peyronie’s Protocol

The most appropriate application of shockwave therapy for Peyronie’s disease is during the active phase, when several of its strongest effects are most relevant. Understanding the active and stable phases of Peyronie’s disease can help explain why treatment approaches differ over the course of the condition.

Pain reduction during active-phase disease allows men to maintain sexual function while awaiting disease stabilisation. Improved vascular health in the surrounding penile tissue supports erectile function. Potential plaque modulation during the period of ongoing collagen deposition is biologically plausible even if the curvature evidence is mixed.

In the stable phase, shockwave therapy is less central to curvature management but remains relevant for men with significant concurrent vasculogenic ED, which is common in Peyronie’s patients given the shared vascular mechanisms. A man with stable Peyronie’s disease and erectile dysfunction that limits his sexual function is a reasonable candidate for shockwave as part of a broader ED treatment approach, independent of its effect on curvature.

What to Expect from Shockwave Therapy for Peyronie’s Disease

Men who undergo shockwave expecting curvature straightening are likely to be disappointed. Men who undergo it understanding that the primary goals are pain reduction, erectile function preservation, and plaque stabilisation during active disease are more likely to have an experience that aligns with what the evidence supports.

Shockwave therapy is not a substitute for a comprehensive Peyronie’s assessment and a treatment plan that accounts for disease phase, curvature severity, erectile function, and the patient’s own goals and preferences. It is one tool in that plan — a useful one in the right context.
